Purim 2021 Megillah Readings Registration

This form should be filled out individually for those attending Main Sanctuary and Women's megillah readings, and as a family group for those attending family megillah readings in the social hall.
Fast of Esther/Erev Purim, Thursday February 25
Purim Day, Friday February 26
Guidelines and Restrictions:
  • A face mask must be worn for the entire time that you are in the building. 
  • Once you enter the shul you will be directed to a washing station where you must wash your hands with soap and water according to CDC guidelines. You will then be shown your assigned seat. 
  • You may select a megillah from the table as you enter the shul  or bring your own. 
  • If you are not feeling well for any reason or have any symptoms (fever over 100, sinus pain, persistent cough, reduced or altered sense of smell/taste, stuffy nose, chills, fatigue, sore throat, joint pains, or diarrhea) you should not attend.
  • Social distancing of 6 feet must be maintained between individuals or families at all times while in shul.
  • Relatives and guests who are traveling from any state on the Ohio Watch List ( https://coronavirus.ohio.gov/wps/portal/gov/covid-19/families-and-individuals/COVID-19-Travel-Advisory/ ) must quarantine for 14 days before coming to shul.  If they are staying at your home this extends to every member of your  household.  The same applies if you or any member of your household has visited a state on the watch list within the previous 14 days.
  • Plan to arrive at shul 15 minutes early to allow for the check in process.  Upon arriving, please follow all directions from the staff member and/or rabbi.
